2009 Underwriting Information Report

INSTRUCTIONS


First Time 2009 Access

If you are accessing the 2009 Underwriting system for the first time, some of the Underwriting information submitted during last year's survey will be automatically completed for you to aid in data entry. Should you have any modifications to this data, it will be preserved when you save your 2009 data.

Please complete the following underwriting information and submit it online by 5:00 p.m., March 24, 2010.

You may enter and save partial information. To do this, enter the information you have and click "Save Current Data." The information you entered will be saved until the next time you open the form. After you have completed the entire form, indicate so by checking the "Submission Final" box below and then click "Submit".

(NOTE: The finalize button will only be available after all other sections have been completed in their entirety.)

Checking the box will indicate to staff that you do not intend to make further revisions to your entries. E-mail a copy of the 2009 IRS Form W-3, or summary report from your payroll processing company for your employees, Redevelopment Agency and other covered agencies, if any, to the CALIFORNIA JPIA offices. The form submitted should state payroll for the calendar year of 2009.
