| The State of California requires evidence of financial responsibility be carried at all times in all vehicles. The Authority, in accordance with the provisions of §16051 of the California Vehicle Code, has provided the form below for that purpose. The agency clerk should complete and sign a form for each vehicle. You may laminate them if necessary, and place them in all city vehicles. |

| The Authority has also developed a Driver’s Report of Accident form to be kept in the glove compartment of all agency vehicles, and is provided below. Members are encouraged to use the Driver’s Report of Accident form anytime agency vehicles or personal vehicles are being used for official business and are involved in an auto accident. Completed Driver’s Report of Accident forms, including the witness and information pages, should be submitted to the Authority’s third party administrators as claims are filed by the agency. |
