The State of California requires evidence of financial responsibility be carried at all times in all vehicles. The Authority, in accordance with the provisions of §16051 of the California Vehicle Code, has provided the form below for that purpose. The City Clerk should complete and sign a form for each vehicle. You may laminate them if necessary, and place them in all city vehicles. The Authority has also compiled Auto Accident Kits to be kept in the glove compartment of all agency vehicles. The kits contain a Driver's Report of Accident form, information exchange cards, witness information cards, and claims reporting procedures. The kits are available in English and Spanish.
