Evidence of Coverage, sometimes referred to as a Certificate of Insurance, provides documentation of liability and/or workers' compensation coverage, and is commonly issued to third parties, such as individuals, vendors, companies, schools and/or other agencies.
We will endeavor to provide this to you within two business days of receiving necessary documentation, including the front and back of any written statements or agreements, such as contracts and permits.
If you select to name a third party as an additional Protected Party, you must provide an executed copy of the written agreement within two business days of this request. If this document is not received, we will be unable to process the request and it will be closed. The agreement, and any other supporting documentation can be uploaded on the next page, sent by email by replying to the confirmation email you receive when you click Submit Request or by fax to (562) 924-0101.
By following the steps below, Members can request an Evidence of Coverage letter to be issued to a named party.
If your request is urgent please contact Edith Aviña (562-467-8776) or
